Fill out the intake forms that were sent to you after scheduling your appointment. This will provide us with all the necessary information to assist you.
Intake
We will complete a thorough exam including: health history, range of motion, movement assessment, local exam, orthopedic, and neurological testing.
Exam
We will go over your diagnosis and individualized care plan for your specific condition. We will also educate discuss all diagnoses and treatments with you.

This allows us to focus on providing the best treatment for you, without being limited by insurance guidelines and contraits. We work with all insurance plans, but do not bill directly. Instead, we provide the necessary paperwork for you to submit to your insurance for reimbursement. If you're unsure about your Out-of-Network benefits, contact your insurance provider to learn about chiropractic coverage. Our priority is to give you the care you deserve, tailored to your specific needs.
